Your claims

At Queensland Country we understand that receiving reimbursement of claims you have paid is important, therefore we give this our top priority. There are many different ways in which you can submit a claim to us. These are listed below:

 

1. Point of Service

A claim can be made with a health care provider who has an electronic claiming facility. The system processes your claim much like an EFTPOS transaction and allows you to just pay the difference, and this convenient method finalises your claim on the spot without leaving the surgery or practice. It is simply a matter of swiping your Queensland Country Health Membership card.

 

2. Face to Face

Queensland Country Health Fund has dedicated service centres available for Members to visit for any enquiry and staff at these locations can also process your claim, most times while you wait. Simply present your provider account/receipt and we will do the rest.

 

3. Over the Counter

We have a close alliance with Queensland Country Credit Union which means that you can drop off claims to any branch. These claims will be forwarded to the Health Fund for processing.

 

4. Claiming by fax

We provide the option of faxing your claim to us for payment.  Please download, complete and sign this claim form, and fax to us on (07) 4725 7377 along with relevant accounts and receipts. Please retain your original receipts as they may be requested.

 

5. Claiming by email

We are happy to accept a scanned and emailed copy of your claim for payment. Please email through your claim form, and the relevant accounts and receipts to us at qchealth@qccu.com.au. Please retain your original receipts as they may be requested.

 

6. Claiming by mail

Members are welcome to mail to us any claim for payment. Please mail to us your claim form, and the relevant accounts and receipts to the following address. Please remember to keep copies of your paperwork.
