WHAT YOU'LL NEED
Joining our health fund is simple, but there are a couple of things you will need before you get started...
- Your Medicare card
- Date of birth and details of everyone to be covered
- Bank account details to set up a direct debit for payment of contributions
- Policy details of your existing health fund
- Transferring from another health fund?
Please complete a Clearance Certificate Request. This authorises your other health insurer to terminate your policy and provide details to us about your membership. Don't forget to cancel any direct debit arrangements you may have with this health fund.
- Claiming the Government Rebate as a reduction in your premium?
Please complete the Application to receive the Federal Government rebate.

TIME TO JOIN
Once you've got that information, simply choose one of the following way to join our health fund and you're on your way to accessing great health cover.
AFTER YOU APPLY
On receiving your application, we will contact you within 2 working days. We'll send you a welcome letter within 7 days and your new Member card will follow a week later.
Your Membership will commence from a date in the future that you nominate, or the date that your application is received by Queensland Country Health Fund or Queensland Country Credit Union.
